COVID-19 APPOINTMENT PROTOCOL
Please review the following steps to be taken before, during, and after each inperson appointment at Synergy Health Centre. We appreciate your willingness to follow these standards and promote health and safety for all!
On the day of your appointment, we will require you to fill out an online form no
more than 2 hours before your appointment. This form includes a health
screening questionnaire ensuring you are an appropriate candidate for in-person
care, and are comfortable with the risks associated with an in-person appointment.
You will not be able to enter the facility until this form is completed. Please note that
you will need to repeat this form before each consecutive appointment. If you do not
see this form in your inbox the day of your appointment, please check your junk
folder and contact our clinic if it’s still missing.
All clients who enter our space are required to be wearing a mask.
When you arrive at the clinic, please wait in your vehicle until the time of
your appointment. Our front desk staff will call you when they are ready for you to
come in for your appointment. Please be sure to provide our administrative team
with your cell phone number at the time of booking.
Please leave all personal items (including jackets, water bottles, etc.) in your vehicle. If
you will be arriving by bicycle, we ask that you lock your helmet to your bike, and
leave any other personal items at home.
To ensure your safety and protection we are controlling the use of the washrooms in order to ensure sanitization occurs between each usage. Please always to notify one of our team members after using the washroom. In some cases you may be required to get a key from us.
